"Family Physician Checkup" for the Body of Christ

Introduction: Paul wrote: "For just as the body is one and
has many members, and all the members of the body, though many, are one
body, so it is with Christ. For in the one Spirit we were all baptized into
one body ... Indeed, the body does not consist of one member but of many ...
you are the body of Christ and individually members of it" (1
Corinthians 12:12-27). This questionnaire will be a "family physician
checkup" on the body of Christ within your congregation. It will be used to
help your church leaders set priorities for your congregation to reach more
people and to invite new disciples to follow Christ. After all the
questionnaires are tabulated, a report and "prescription" will be shared
with church leaders.
This process will help your congregation determine how best to respond to
Christ's call which is not only to be faithful - but also to be fruitful
disciples. Jesus said: "I am the true vine, and my Father is the vine
dresser. Every branch of mine that bears no fruit, he takes away, and every
branch that does bear fruit he prunes, that it may bear more fruit ... He
who abides in me, and I in him, he it is that bears much fruit ... By this
my Father is glorified, that you bear much fruit, and so prove to be my
disciples." (John 15:1-8)
This questionnaire will help your congregation determine how best to respond
to Christ's two great commissions in Matthew: "Go therefore and make
disciples of all nations" (Matthew 28:19), and "I was hungry and you
gave me food, I was thirsty and you gave me drink, I was a stranger and you
welcomed me, I was naked and you clothed me, I was sick and you visited me,
I was in prison and you came to me ... as you did it to one of the least of
these, you did it to me." (Matthew 25:35-40).
Instructions: Please answer the following questions with your
personal opinions. Do not respond with what you think others in the church
may feel. Give your ideas and perceptions. Your responses will be kept
confidential. Please complete the survey in one session within two
weeks of receiving the invitation to participate. There are 80
survey questions. Allow yourself at least 45 minutes.
Important note: If you are entering more than one survey at a time, you must close your browser after each submittal and come back to this page; otherwise, the subsequent surveys will not be recorded.
